14U Spring Tryouts

It is the clear understanding of each of the undersigned that all medical, financial, and legal liability issues, activities, and consequences are the exclusive responsibility and domain of the player and/or his respective family, parent(s), and/or guardian(s). I agree that at no time shall Diablo Valley Oaks nor any of its associated personnel, including, but not limited to, founders, managers, coaches, board of directors, et al. (collectively, "Agent") be sued legally or equitably (individually, collectively, or organizationally) in any manner, for any expenses, damages (perceived or real, intentional or unintentional), losses, injuries, complaints/problems, complications, and/or inconveniences which may result from participation with team activities and/or team initiatives. I agree to release and forever hold harmless Diablo Valley Oaks and its Agents in all matters, legal, medical, personal, financial, and/or otherwise. NO CLAIMS SHALL BE MADE FOR ANY REASON. I have read, discussed, and understood the CODE OF CONDUCT AGREEMENT, and fully and unequivocally support it and its intent. We are considered SECONDARY if any insurance claims are made.

I understand that this tryout date and time is MANDATORY. We recognize that some players may have other sports or commitments with another club; however, in order to be considered for the fall team, attendance at this tryout is required. A great deal of preparation goes into these tryouts before the actual date to ensure the best possible experience for your player and to give them every opportunity to showcase their talents. When players do not attend without communication, it makes evaluations and team planning very difficult. If, for any reason, you experience an issue during sign-ups or a conflict arises, STOP and reach out to us immediately so we can assist.
